- Page 1 - DOE Cover page
- Page 2 - Cover page supplement listing in table form the name/affiliation/position for each US and Japan person
- Page 3 - Cover page supplement listing in table form listing the budget request to DOE and KEK for 3 years
- Page 4 - ½ page Introduction, Objectives, Proposed Research Methods
- Page 4-10 - propose N topics, each topic enumerated and underlined, with ¼ to ½ page of 4-8 sentences of very succinct statements of what is important, and listing what each lab would be doing under that topic
- Page 11 - One page table of activities, all N topics w/ sub-tasks, showing which FY quarter each happens within
- Page 12-14 - Separate institutional responsibilities, each topic enumerated and underlined, then each institution listed and underlined with text 1-4 sentences describing exactly what will be delivered

- 11/30 Wednesday : Budget finalized and approved by PAO
- 12/02 Friday : Appendix 1 and 2 complete at the minimum
- 12/05 Monday : All Appendix complete
- 12/07 Wednesday : Narrative 75% complete
- 12/08 Thursday : Proposal package 95% complete submitted for internal review
- 12/08 – 12-14 : Finalizing of proposal, work on feedbacks from PAO, enter into PAMS
- 12/15 : DOE deadline
